• Birth Certificate

• Immunization Record

• School Physical

• Parent Drivers License

• Proof of Residency

You must provide the following documents for Proof of Residency:

• Current Mortgage Statement or Lease Agreement

• Current Utility Bill 

Plus one of the following showing your address:

• Bank Statement

• Insurance Policy

• Pay Check Stub

• Credit Card Statement

• Property Tax Record

• Vehicle Registration 

Additional documents that may be uploaded:

• Custody Documentation

• Social Security Card

• Controlled Open Enrollment Application

• VPK Certificate 

Parent or guardian signature is required before final submission.
